2. Notice of Alleged Infringement (Takedown)

To submit a DMCA takedown notice, send a written notice to the Designated Agent that includes:

A physical or electronic signature of the owner or authorized agent.
Identification of the copyrighted work claimed to have been infringed.
Identification of the allegedly infringing material and information reasonably sufficient for us to locate it (e.g., URL).
Your contact information (address, telephone, email).
A statement that you have a good-faith belief that the use is not authorized by the copyright owner, its agent, or the law.
A statement, under penalty of perjury, that the information is accurate and that you are the owner or authorized to act on the owner’s behalf.

3. Counter-Notice

If your content has been removed and you believe the removal was a mistake or misidentification, you may submit a counter-notice with the same elements (signature, identification of removed material and its location, statement under penalty of perjury that you have a good-faith belief the material was removed by mistake or misidentification, your contact details, and consent to the jurisdiction of the federal court for the judicial district where you reside or, if outside the U.S., the U.S. District Court for the Middle District of Florida (which encompasses Pinellas County, Florida, where JYV STREAM LLC is located)).

4. Repeat-Infringer Policy

We terminate, in appropriate circumstances, the accounts of users who are repeat infringers.

5. Misrepresentations

Under 17 U.S.C. § 512(f), any person who knowingly materially misrepresents that material is infringing, or that material was removed by mistake or misidentification, may be liable for damages.
